Formal notification about a cyber incident

What has happened?

On Wednesday 22 November 2023, the Quantum Radiology Group became aware of a cyber incident where an unauthorised third party gained access to our IT systems and encrypted the contents of those systems.

As soon as we detected the incident, we took steps to contain it, and appointed forensic specialists to investigate what had happened, engaged by our legal advisors along with other cyber incident response specialists.

Since the incident we have also:

  • Informed referring practitioners and asked for their assistance in notifying patients.
  • Informed current and former employees and provided them with recommended steps to help protect their personal information and reduce the risk of harm

Out of an abundance of caution, we took our systems offline and closed our clinics. We look forward to re-opening as soon as possible, and opening dates for each clinic will be posted on our website.

We sincerely apologise for any inconvenience or concern that this incident may have caused.

Why are we telling you?

This notice is for:

This notice contains information about what data has been affected, the support available to potentially impacted individuals and immediate actions that you can take to help protect yourself from the risk of scams or other potential harms.

  • Quantum patients (or parent/guardian) who may be concerned about the potential for their information to have been impacted;
  • Referring practitioners who have referred patients to a Quantum clinic;
  • Former and current employees who work or who have worked at Quantum; and
  • Other individuals, including contractors, who may have shared personal information with us directly or whose personal information may have been provided to us indirectly by another health service provider (such as another imaging business).

What information has been impacted and steps to take?

Based on our investigation, the unauthorised third party may have accessed personal information of individuals as detailed in the table below.

Please note Quantum does not collect or store credit card details.

For any other queries relating to the cyber incident, not answered by the FAQs, we have set up a support centre to respond to and assist you with any queries you still may have about the incident. The support centre will be available from Friday 15th December, contact details as follows:

Referring Practitioners• Referring practitioner name, title, specialties and provider numbers; and
• Medical centre contact details including phone numbers, addresses, email and fax number.

For clinical and other matters unrelated to the cyber incident please contact the relevant Quantum clinic.
Current and former employees• Employment contract and related documents (including signatures, registration and licensing information such as AHPRA registrations, radiation licence numbers)
• Payslips with detailed pay information (including salary, bonuses, child support deduction information)
• Banking details for payroll
• Tax file number and declaration form, including date of birth
• Superannuation details and declaration form
• Personal contact information, including emergency contacts
• Covid vaccination certificate
• Salary sacrifice forms
• Centrelink Reference Number e.g. for the purposes of paid parental leave
• Any other personal information you may have provided us with including:
• Health information including medical certificates, injury details; and
• Information for staff on sponsored employment types (E.g. passport and visa).
We have sent communications to current and past employees via email and post to advise them of steps to take.

In addition to the steps highlighted above, we encourage you to take the following precautions:

  • Remain alert to any suspicious email, SMS or telephone communications that are disguised to look like they come from someone you know or trust, including Quantum Radiology.
  • Verify the legitimacy of communications by authenticating the sender. Do not respond to the suspicious communication but rather contact the person separately via their usual means of communication to verify the legitimacy of the communication.
  • Do not open links that look suspicious and be alert to phishing scams. This could include scams that target you through post or email. Phishing scams are attempts by scammers to trick people into providing their personal information, passwords, credit card numbers and/or sensitive personal information.

Authorities notified:

We have notified and are working closely with the government and relevant authorities, including the Australian Cyber Security Centre, the Office of the Australian Information Commissioner, the Australian Tax Office as well as NSW and Commonwealth Government bodies. We have also notified law enforcement. We will continue to keep the government and relevant authorities appropriately updated, and continue to work with the NSW Government and the Commonwealth Cyber Security Response Coordination Unit.
